The Complete pre-operative examination

It was a lifelong dream to have rhinoplasty. A beautiful-looking nose to improve my breathing and my confidence.

One patient’s experience of the full and thorough pre-operative examination carried out by Dr. Mireas.

She agreed to let us photograph her first visit to the VIP-Rhinoplasty Center to help us explain what should happen before a patient takes the decision to go ahead with a rhinoplasty procedure, and describes the experience in her own words.

Rhinoplasty on a deviated nose with ultrasonic piezo by Dr. Mireas

For as long as I can remember, I've wanted to have a nose job. To get a beautiful-looking nose to improve my breathing and my confidence.

When I visited Dr. Mireas, after he had recorded my medical history, he listened carefully to my concerns and what I wanted.

He then examined me using the state-of-the-art equipment in his private practice, and described what exactly the process of rhinoplasty surgery involved and what he recommended in my particular case.

My discussion with the doctor lasted a whole hour in total and during it he answered all my queries so that I left reassured, with all the necessary information about the operation.

To tell the truth, I was surprised by how much time he spent with me, but as he explained, a demanding operation like rhinoplasty can't be analyzed properly in a quick visit lasting ten or fifteen minutes.

The first examination:

My examination by the doctor was as follows:

Medical history

At the start of the visit, Dr. Mireas recorded my medical history.

He asked about my age, past and present medical conditions, daily habits such as smoking and drinking, possible hereditary diseases, any medication I’m on or medicines I take occasionally, and so on.

He then made a physical examination of the inside of my nose by endoscopy, recording the results on his computer.

Endoscopy

After the initial examination, Dr. Mireas showed me exactly what the inside of my nose looks like on his computer, and explained that one reason why I had trouble breathing, apart from a deviated septum, was inferior turbinate hypertrophy (when the bony structures inside the nose are too large), and that dealing with it would help improve my breathing.

The solution was rhinoplasty (functional and cosmetic) which would finally help me breathe properly and have a beautiful-looking nose.

Rhinoplasty simulation: I saw my ‘before’ and ‘after’ face in three dimensions

After the analysis of the problems inside my nose, we began a 3D simulation for a cosmetic analysis of the operation.

This took place using the Vectra 3D/M3 System (Canfield Scientific, NJ – USA). The particular system – according to my research in the internet- is consider to be the most advanced in the whole world and  it is available in Greece exclusively at the VIP-Rhinoplasty Center.

I was worried about how I would look after my rhinoplasty. And I really wanted to see the results in advance.

The process was very simple and only lasted a few minutes. I sat in front of a camera, the flash went off, and within a minute I could see my face in three dimensions on the computer screen.

The discussion with Dr. Mireas

I began to explain to the doctor how I wanted to look…

“First of all, I want to be able to breathe better... I also want my nose to be straight, without a curve or a bump, and for the nostrils to be a bit smaller

And most importantly of all, I want my nose to look natural and not like it has been operated on.”

The doctor then explained exactly what was possible from everything I'd said, as well as what would go best with the rest of my facial features.

He showed me the changes that I'd described to him one-by-one on his computer screen. For the first time, I saw my nose changed in exactly the way I’d dreamed of and by the end of the process I could see the final result and was able to compare my face before and after so that I could be certain about the result.

The simulation was really impressive and I could see what I wanted most of all - a completely natural-looking result.

I know that all patients have their own individual demands, but by using 3D simulation, a specialist surgeon like Dr. Mireas can definitely explain all the possible options to them and show them the final result, which I can really say was exactly what I saw in the mirror after the operation.

I chose rhinoplasty using Piezosurgery

After my visit to Dr. Mireas I decided to go ahead with the rhinoplasty.

I booked a date for the operation and all I had to do was not eat or drink anything for a few hours beforehand.

I did a rhinoplasty in combination with correction of a deviated septum and cauterization of the turbinates, so that I could breathe better, ending up with a natural, beautiful-looking nose that goes perfectly with the rest of my facial features.

My next visit was 6 days after the operation. The doctor examined me carefully and removed the splint so that I could see my new nose for the first time.

It looked so beautiful and went so well with my face…

Thank you so much, doctor!

Frequently asked questions about the pre-operative examination

How should I prepare for my pre-operative examination?

You don’t need to do any special preparation before you are examined.

It is a good idea, though, not to wear heavy makeup that changes your general features too much, so that the simulation can be as accurate as possible.

Will I have to answer questions about my medical and general history?

For reasons of safety during the operation, you will be asked about health problems, allergies to medicines, and previous medical treatments, as well as any use of medicines, vitamins, nutritional supplements, tobacco, alcohol, and drugs.

You will also be asked about any previous surgeries you have had.

You must give your surgeon accurate information about all of the above for your own safety and the optimum outcome.

Can I wear makeup during my visit for the rhinoplasty simulation?

The simulation is not affected by light makeup, and you can wear it without any problem. However, in order for the simulation to be accurate, you should avoid heavy makeup that changes your general features and makes it difficult for problems to be identified.

Is ultrasonic rhinoplasty performed with open or closed surgery?

Piezo ultrasonic rhinoplasty is the safest and most modern surgical method and can be both open and closed. It offers the surgeon perfect visibility and access and allows the nose to be sculpted with absolute precision, delivering the best possible result to the patient.

What is open rhinoplasty?

Open rhinoplasty is where small incisions are made inside the nose, as well as a very small incision in the skin between the nostrils. This incision heals extremely well and is not actually visible a few months after surgery.

What is closed rhinoplasty?

In closed rhinoplasty, all the incisions are made inside the nostrils, with no incision being made in the skin, so there is not even the very small scar between the nostrils that is left in open rhinoplasty.

Which method is considered safer, open, or closed?

We can choose either open or closed access, depending on the anatomy of the patient’s nose, the problems being dealt with, and the desired results, as the case may be.

In all cases, the use of the ultrasonic system ensures the absolute precision required for the best aesthetic and functional outcome. We will choose the method that ensures the best possible results depending on your situation.
